Cruise Ship Outbreak: Hantavirus Claims Three Lives, Investigation Underway

Albuquerque: A suspected outbreak of hantavirus aboard a cruise ship has resulted in three fatalities and several illnesses. The World Health Organization (WHO) has confirmed that hantavirus has been identified in one of the cases, prompting ongoing investigations and laboratory testing related to the outbreak.

Cruise ships are known for being crowded environments, often leading to the spread of gastrointestinal and respiratory illnesses. Typically, these outbreaks are linked to highly contagious pathogens, with norovirus being the most prevalent. According to the U.S. Centers for Disease Control and Prevention (CDC), there were 23 gastrointestinal outbreaks reported on cruise ships visiting U.S. ports last year, with 18 attributed to norovirus. However, hantavirus is not commonly associated with cruise ships and is not easily transmitted between individuals.

Historical Context of Hantavirus

Research indicates that hantaviruses have existed for centuries, with documented outbreaks in Asia and Europe. The emergence of a previously unknown group of hantaviruses in the early 1990s in the southwestern United States led to the identification of hantavirus pulmonary syndrome, an acute respiratory disease. This illness gained notoriety in 2022 when Betsy Arakawa, the wife of late actor Gene Hackman, succumbed to a hantavirus infection in New Mexico.

Hantavirus is primarily transmitted through contact with rodents or their excreta—urine, saliva, or droppings. The risk of exposure increases when these materials are disturbed, allowing virus particles to become airborne. Individuals often encounter hantavirus in homes, cabins, or sheds, particularly during cleaning activities in poorly ventilated spaces or when handling areas contaminated with rodent droppings.

Steven Bradfute, an associate professor and associate director of the Center for Global Health at the University of New Mexico Health Sciences Center, is involved in sequencing efforts to better understand hantavirus. He noted that while human-to-human transmission of hantavirus is rare, it can occur. A strain endemic to Argentina and Chile has the potential for person-to-person transmission, but it is generally limited to close contact scenarios, such as sharing a bed or food.

Bradfute emphasized that there has not been a significant spread of hantavirus infections among people historically, and there is no current indication of a large outbreak stemming from this case. The CDC began monitoring hantavirus following a notable outbreak in 1993 in the Four Corners region, where Arizona, Colorado, New Mexico, and Utah converge.

Symptoms and Progression of Hantavirus Infection

Most cases of hantavirus pulmonary syndrome manifest symptoms within one to eight weeks after exposure to an infected rodent. Initial symptoms may include fever, chills, muscle aches, and headaches, resembling flu-like symptoms. As the infection advances, patients can experience severe respiratory distress, characterized by chest tightness and fluid accumulation in the lungs.

Another syndrome associated with hantavirus is hemorrhagic fever with renal syndrome, which can lead to bleeding, high fever, and kidney failure. The fatality rate for hantavirus pulmonary syndrome is approximately 35%, while the death rate for hemorrhagic fever with renal syndrome varies between 1% and 15%, according to the CDC.

Hantavirus on Cruise Ship

Currently, there is no specific treatment or cure for hantavirus infections. However, early medical intervention can significantly improve survival chances. Despite extensive research, many questions remain unanswered, including the reasons for varying severity of the disease among different individuals and the mechanisms behind antibody development. Researchers continue to monitor patients over extended periods to identify potential treatments.

Bradfute remarked that while hantavirus infections are serious, they remain relatively rare in the Americas. This rarity has historically led to less research funding, although interest in hantavirus studies has increased recently. Harkins noted that rodent exposure remains a critical factor in understanding the disease.

Preventive Measures Against Hantavirus

To minimize the risk of hantavirus infection, public health experts recommend reducing contact with rodents and their droppings. Protective gloves and a bleach solution should be used when cleaning rodent droppings. Experts advise against sweeping or vacuuming, as these actions can aerosolize virus particles.

Hantavirus encompasses a broad family of viruses, with various strains found in different regions. Most strains do not transmit between humans, with the potential exception of a South American variant. Given that the cruise ship originated in Argentina, this strain is under scrutiny. Angela Luis, a hantavirus researcher at the University of Montana, emphasized the importance of a thorough investigation to clarify the circumstances surrounding the outbreak and to determine if human-to-human transmission is possible.

Luis expressed optimism about the potential for new insights from this investigation.
